dipertanggungkan oleh produk asuransi.<\/b><\/p>\n<p><span style=\"font-weight: 400;\">Untuk melakukan Klaim produk asuransi Jaga Aman untuk risiko meninggal dunia akibat kecelakaan, Tertanggung atau Ahli Waris harus melakukan Klaim paling lambat 30 hari kalender sejak tanggal terjadinya hal yang menimbulkan Klaim dengan mengikuti prosedur Klaim sebagai berikut.&nbsp;<\/span><\/p>\n<ol>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Formulir Klaim Meninggal Dunia<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Fotokopi Identitas diri (KTP) dari Pemegang Polis, Tertanggung, dan Ahli Waris<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Fotokopi Kartu Keluarga yang dilegalisir dan\/atau barcode dari Pemegang Polis, Tertanggung, dan Ahli Waris<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Fotokopi buku rekening Bank (halaman keterangan nomor rekening) atau tangkapan layar halaman informasi nomor rekening (untuk tabungan digital) dari ahli waris yang ditunjuk<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Fotokopi Surat Akta Kematian dari Dinas Kependudukan dan Pencatatan Sipil (DISDUKCAPIL)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Fotokopi Surat Keterangan Kematian dan\/atau Sertifikat Medis Penyebab Kematian dari Rumah Sakit yang dilegalisir&nbsp;<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Fotokopi Surat Keterangan Pernyataan Ahli Waris yang dilegalisir<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Fotokopi Surat Keterangan Kepolisian atas peristiwa kecelakaan yang asli atau dilegalisir&nbsp;<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Dokumen lain sebagaimana diperlukan Penanggung apabila Klaim yang diajukan memerlukan klarifikasi dan\/atau informasi lebih lanjut.<\/span><\/li>\n<\/ol>\n<p><span style=\"font-weight: 400;\">Untuk klaim Santunan Perawatan Rumah Sakit akibat Kecelakaan dilakukan secara reimbursement.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Selalu simpan dokumen asli jika suatu saat diperlukan untuk melengkapi dokumen
